It was put to him that Claire Byrne tested positive yet had no fever but Dr Holohan said that the majority of cases would present with fever. My concern would be missing asymptomatic or mild cases but I suppose we have to trust the experts.
I saw that and got the impression that they are concerned given the market demand for tests that approximately 93% of tests were being used on the worried well and that the new criteria should cut waiting times for those who actually have covid 19.

One of the other experts with him also pointed out that the viral load in people who are asymptomatic might be too low for the test.

Primetime also reporting that two cases ( I think in the states) who were practically in a coffin had been treated with HIV and/or malaria drugs and had fully recovered. I also saw a doctor on BBC news asking people not to look for prescriptions for these drugs as they would be needed for the most serious cases .

The Dow jones had its biggest one day gain ever yesterday. The size of the US rescue package has been knocking around since the weekend so you would have to wonder other good news has filtered through to the markets.

Maybe they should check if any of these folks were buying .

We have tested 18k people with approx 7.3% confirmed. Based on the SK numbers we want to get that around 2.5% but we're trending in right direction.

SK have tested 320k people since Jan 20th, we have tested 18k since Feb 29th, proportionally that equates to around 190k tests.

Again it's not necessarily linear, 70% of cases concentrated in one region of SK representing 5% of their population but we're still ramping up.

Looking to countries with similar resources, demographics and timeline so Norway(3 days ahead) 2,779 cases with 12 deaths and Denmark(2 days ahead) 1,591 cases with 32 deaths.

Lots of room to improve on speed on testing but we're managing things well it appears.
HSE are going to cancel all referrals for testing today and change the criteria from fever or cough to fever and cough. Morons. Struggling to handle the testing load and making a sub optimal decision as a result.

That insinuates that the task at hand is somewhat simple.

They have 40k people on a waiting list for testing. Again, proportionally that's greater than the summed total that SK tested since it's first infection 9 weeks ago.

We'll get through the backlog by next week which is far faster than SK got through a much smaller number in almost twice the length of time. 5 weeks to test 40k people, proportionally that's around 400k people in SK, a number SK haven't hit in 9 weeks of testing.

As previously mentioned they're still ramping up. This is a short term action till all 50 testing centres are fully operational. It's not a sub optimal decision. It's an optimal decision given finite resources.

Lloyd, it's not perfect, it was never going to be but they're playing a blinder.

Actually, it’s way worse than assumed. They have laid down extremely stringent criteria that will see most “mild” cases not qualify for a test. This will dramatically cut down on tests. Absolute idiots. That were doing the right thing. GPs distraught over this.

You haven’t seen the directive issues to GPs. There’s no backlog. All referrals on the backlog are cancelled. New strict referral criteria enforced.

I really don't think they have other options.

Well they've concluded 18k tests in 24 days. SK concluded 320k tests in 65 days. We have more proportional tests waiting on results too.

It's clear we have shown the ability to test at scale. They may have changed the criteria so to prioritise testing again given finite resources but let's pretend they're not testing at an extraordinary rate.

Just to give some idea of the scale of requests. There are 20k tests being requested every day, no country has shown a capacity to manage that amount of tests. Like asking Germany to test 360k per day. Their weekly capacity is 160k.

HSE have delivered on everything to date and they're aiming to have capacity of 15k tests per day within the next 4 weeks.
That's 10x the South Korean capacity in a shorter length of time from first confirmed case.

It's a short term solution but clear we are a global leader in testing capabilities to date and goals moving forward.

I thought it was €30 a call, and unless they're a part-time GPs half will go back in tax. You expecting them to work for free?

This is pretty much what happened with my GP nearly two weeks ago. He said, just assume you have it and self isolate. If you need a note, here's a note but the treatment will remain the same.
At the time I was pissed but thinking about it, we were testing 2000 people per day last week but adding multiples of that a day to the backlog. That means we were still not finished testing people from the first day of testing 9 days ago. Given that my symptoms are all but disappeared and my wife (who got a test appointment) is still waiting for a test (and shes the same), he did the right thing.

case numbers, in my opinion, mean fuck all. 90%+ of cases are mild. ICU cases and deaths is the proper number to focus on.

If you have ANY symptoms self-isolate, even if you havent been tested. Ring the doctor and get a note if you need.

Well the ability to inhabit all sides of the argument is great altogether.

If we were lauding our testing approach yesterday, which the thread was, then we shouldn’t be okay with an extreme restriction on testing criteria. The flow chart sent to GPs after midnight last night will mean that the testing net contracts in an extreme way. How can ye have been supportive of the South Korea like approach yesterday yet support this at the same time?

Retain the backlog; continue building capacity; prioritise testing based on at risk criteria; eventually you will catch up and have accurate information that will allow you to make bigger more complex decisions in 3 weeks time. That is the correct approach. Not giving up on testing now.

This is such a gross misrepresentation of what is actually occurring, it's truly startling.

Clearly the queue will rebuild with greater structure around prioritising those of greater risk.
This simply allows it to rebuild from the bottom up opposed to sifting through the backlog to prioirtise retrospectively.
It allows the GP to make the initial decision under the new guidelines and allow further decisions to occur upstream.

It's so self-evidently the logical thing to do that I find it hard to believe anyone would favour the alternative.

I've outlined the numbers and why 20,000 requested tests per day is a phenomenal and how it's not even close to being replicated proportionally in any other leading country to date.

To be, testing serves two purposes, identifying those who might die, and those that might spread it.

Is it not almost more important to catch the asymptomatic cases? If someone ends up care you don't need to confirm the case, but the asymptomatic cases are out spreading it unknowingly?

TBH, how the prioritisation is decided is actually of little interest to me, my interest was more around the idea that testing was being given up on and that being unable to test 20k people a day was some type of failure. Obviously neither are true.

As CMO mentioned last night contact tracing of associated cases on average down from 20 to 5 so I guess that's a decent indicator of how we are reaching those who are asymptomatic. Net is closing around most making it easier to achieve.

The liquidity position of the main listed lessors would appear to be sufficient to manage short term issues - lease holidays with lessees (assuming both some degree of catch up towards year end, and some longer term defaults/repossessions).

If the fallout from Cv-19 is worse, they may face a severe liquidity crunch but in that scenario virtually every airline will need nationalisation to continue in any event.

I assume that could all happen whilst shareholders are wiped.

Lessors are senior secured creditors, they’ll be the last to take a hit, so if you expect AER or AL to be justifiably at 20% of Book, then you should be short all the airlines as they’re going to zero.

The most important reason for testing IMO is to find the most severe cases and treat them accordingly. Yes Johnny down the road might have a mild case of Covoid-19 that doesn't get confirmed but he should be self-isolating as per the guidelines and riding out the illness.

I don't think any country in the world has the resources to catch EVERY case so why not prioritize the most sick and at risk of death?

It's not ours to take and it's now before the courts.

Good stuff. Argue weeks ago that further restrictions were not required, though they inevitably came. Laud high volume testing as our solution, and pivot to a severe restriction of testing that will essentially cut a huge swathe of population from receiving one as the correct and unquestionable policy.

So you can talk about the situation at length if you’re saying what we want to hear basically?

Priority has now been introduced to fix that until more capacity can be delivered. What's the issue?
The way in which they are making GPs prioritise means a huge amount of potential positive cases are missed. This will deny us accurate information about case numbers; percentages of cases suffering severe and critical disease; transmission clusters; etc. Information that will be extremely important in a few weeks when we’re having to decide whether we roll back movement / economic restrictions or not.

The better way was to apply more rigorous prioritisation / triage standards to those referred for testing; ignore complaints about the backlog while you build testing capacity; and eventually get to everyone.
