Just imagine the player to act has called the bet he is facing. That is now the size of the pot. And he can raise that amount. Like in the first example he's facing a bet of 200 to call. He calls making the pot 500 and can now raise 500 making a total of 700(his call of 200 + raise of 500). Its not that confusing once you include the players call making up the size of the pot

Max raise is calculated as in pot total if you call.
When UTG calls, the 200 BB, there is 500 in a called pot, so he then can raise the BB by 500.
When facing 1 pot bet, you always get 2 to 1 on your money. In this case, if folded to the BB, he has to call 500 into a 1000 pot.
The 2xlast bet plus pot, and, 3xlast bet plus pot, are just quick ways of working it out, but not the actual rule. They are both correct, reason being that the 3x doesn't count the last bet as part of the pot and the 2x does.
So for the UTG raiser.
He can make it = (original bet) plus 100+200+200(his call)
2x = 100+200(pot) plus 2x200
3x = 100(pot ex. bet) plus 3x200
It's all the same amount (700), its just how you group the 100 and 200s.
